Valleywise Health Partnership
HOPE recently launched a partnership with Valleywise Health in Maricopa County to support individuals transitioning from incarceration back into the community.
This initiative includes collaboration within Mesa Probation and Parole and focuses on:
- Peer support upon release
- Connection to healthcare and behavioral health services
- Linkage to housing and community resources
- Reintegration planning prior to release
HOPE and Valleywise are also expanding services inside correctional facilities to establish support before individuals return to the community.
The goal of this partnership is to promote stability, empowerment, and successful reentry outcomes for justice-involved individuals.
