Arizona Complete Health (AzCH) & Mercy Care
HOPE partners with Arizona Complete Health (AzCH) and Mercy Care to support individuals newly diagnosed with Serious Mental Illness (SMI) across Arizona.
Through these collaborations, HOPE provides:
- Outreach and engagement
- Peer support services
- Education about member benefits and rights
- Assistance identifying social determinants of health
- Empowerment-focused, person-centered support
The program currently serves members in Northern Arizona, Southern Arizona, and Maricopa County.
