Supporting Transitions from Inpatient Care
HOPE’s PACT Programming in Pima County serves as a second responder program for individuals transitioning from Level 1 behavioral health facilities back into the community.
HOPE partners with:

Peer specialists provide individualized support both inside healthcare settings and in the community to help members:
- Reconnect with health homes
- Access community resources
- Maintain stability after discharge
- Reduce hospital readmissions
PACT programming strengthens the continuum of care by ensuring individuals are not navigating recovery alone after hospitalization.
